WHAT IS APOTHECARE?

Lee’s Inlet Apothecary’s Long-Term Care at Home Program, ApotheCARE, is a benefit that can be offered to patients who, due to age, health or disability may need additional pharmacy support to better manage their medications independently.

When a patient signs up for ApotheCARE their medication refills are synchronized, packaged, and delivered to their home on a regular schedule. They also receive additional clinical oversight by a trained team of pharmacists and pharmacy technicians who will provide extraordinary care and attention to their medication regimen and care plan.

ApotheCARE is designed to help keep patients safe, reduce medication errors, and to help family or home healthcare staff with managing drug therapies. We like to say, “Right Med, Right Dose, Right Time”. The patient’s medication regimen is packaged for individualized dosage, cross-checked for interactions and allergies, and reviewed with the patient and/or their care team on a regular schedule.

WHO QUALIFIES?

Any patient, regardless of age, may qualify for our ApotheCARE service, and most who are discharged from a facility do. When you contact Lee’s Inlet Apothecary, the pharmacist will review a series of questions to determine eligibility.

WHAT DOES THIS COST?

There is no additional cost outside of your chosen Medicare/Medicaid copayments, coinsurance and/or deductibles for qualified patients.  For anyone else outside these plans but would like to participate in our ApotheCARE program can be enrolled for a small monthly fee.

HOW DO I SIGN UP?

Enrolling is easy!  To start the enrollment process simply stop by or give Lee’s Inlet Apothecary a call and let us know that you are interested in ApotheCARE.  We are able to begin enrolling patients in person or over the phone. 

If you are a caretaker and would like to enroll a loved one in the program on their behalf, you may also be able to do so- just ask!
